Biohazard's self-titled debut came out on 30 June 1990 on Maze Records. It is the first of the band's studio albums, and it sits at the start of a catalogue that runs through Urban Discipline in 1992 and on to Divided We Fall in 2025. The sound mixes crossover thrash, hardcore punk and rap metal, a blend that marks the band out from the start.

The record has a cinematic streak. "Retribution" opens with a clip from The Godfather Part II, and "Wrong Side of the Tracks" uses dialogue sampled from The Warriors. Two of its songs, "Wrong Side of the Tracks" and "Hold My Own", were rerecorded for Urban Discipline, so this is the place to hear them first. The original pressing showed an orange biohazard symbol over a circular brick wall pattern, while the 2004 remaster carries a different cover.
